Job Description

Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE’s program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Occupational Therapist, the Certified Occupational Therapy Assistant, (COTA), provides clinical occupational therapy services for Horizon PACE program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home, or in-home setting. Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment for participants.

Position Responsibilities:

* Provides treatment on acute, restorative and maintenance levels to participants.
* Maintains regular attendance at and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
* Notify the Occupational Therapist of any issues or impediments to the participants’ plan of care delivery or any changes in the participant’ s condition.
* Monitor ongoing use of any DME, specialized shoes and prosthetics.
* Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Occupational Therapist.
* Carries out accepted principles and practices of occupational therapy for prevention of functional decline, sensory and cognitive limitations, increase or maintenance of strength and endurance, and provides treatment to increase self -care skills.
* Assists as liaison with the nursing home and assisted living staff regarding occupational therapy needs of participants who are their residents.
* Reports on participant’s progress, documents occupational therapy notes in the participant’s medical record.
* Assists in recommending occupational therapy policies and procedures concerning services provided. Maintains quality assurance program, safety, environmental and infection controls.
* Educates participants and family members in proper care and use of various support and exercise programs and use of durable medical equipment at home.
* Trains family members, other caregivers, and Horizon PACE staff in methods to maximize safety and functional abilities in performing Activity of Daily Living (ADL) skills.
* Assists with data collection and prepares reports. Maintains any necessary participant statistics.
* Acts within the scope of his or her authority to practice.
* Maintains the department in a clean and orderly manner. Keeps equipment in good working condition according to manufacturers’ recommendations.
* Follows all Horizon PACE policies and procedures, and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
* Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.
* Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
* Participate in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
* Participates in and supports Quality Improvement Initiatives.
* Performs other duties as required or requested.

Experience & Education:

* Must have an associate’s degree in an Occupational Therapy Assistant program. Have a current, valid Kentucky state licensure or certification as a Certified Occupational Therapy Assistant, (COTA). Be legally authorized, currently licensed, registered, or certified if applicable to practice in the state of Kentucky.
* One year’s experience working with a frail or elderly population preferred.
